Resident Services Coordinator

JOB DESCRIPTION:

Under the supervision of the Director of Social Work, Program and Support Services, provides effective care and support to both potential and current residents and their families.

RESPONSIBILITIES

Admissions:
Work with the Director of Care to review long-term care applicants and accept or decline applications
Act as primary contact for all applicants and their families. Field all incoming requests for information regarding the admission process and continue to support applicants as they await admission.
Provide tours of the long-term care home (presently virtual tours)
Complete the resident admission process: pre-admission, coordination of admission with Home and Community Care Support Services (formerly known as LHIN), communicate with families and residents prior to the day of admission and participate in establishing a welcoming environment for new residents
Provide admission information to departments as appropriate i.e., need for assistive devices, need for oxygen, psycho-social information pertinent to the success of the admission
Act as a liaison between residents/Substitute Decision Makers/Power of Attorneys and Hillel Lodge staff.
Participate in the transfer, discharge of residents, and occupancy management

Resident and Family Engagement:
Provide support and guidance to residents and their care partners (family & friends) to assist with their integration into the home.
Participate in initial care conferences and facilitate resident’s annual care conferences
Ensures timely communication with residents and family/personal representatives relative to concerns and requests.
Complete summaries of care conferences.
Support Residents’ Council president in developing agenda and facilitating council meetings, as needed

Administrative:
Maintain resident records and documentation for the department in accordance with the facilities policies and procedures
Maintain minutes for weekly interdisciplinary team meetings
Provide administrative support to the Director of Social Work, Program and Support Services
Make referrals to external agencies as required and provide follow up
Meet weekly or as needed with the Director of Social Work, Program, and Support Services
Performs any other duties as required that are necessary for the welfare of the residents and efficient operation of the Home

QUALIFICATIONS:
Degree in Social Work or diploma in social service work or related field (i.e. – gerontology, community services work, counseling psychology, etc.)
Registration with the Ontario College of Social Workers and Social Service Workers (preferred)
Excellent English oral and written communication skills
Diplomatic, with excellent organization and priority management skills
Clinical skills in social assessments, group and individual counseling, and care conference facilitation
Experience working with geriatric populations including those with Alzheimer’s disease and other dementias, depression, responsive behaviors, and other psychogeriatric issues
Knowledge of applicable Ministry of Health and Long-Term Care legislation and standards, including but not limited to the Fixing Long-Term Care Act, PHIPA, Health Care Consent Act, Substitute Decisions Act, etc.
Knowledge of financial resources and benefits to long-term care applicants and residents
Computer literate with an understanding of RAI-MDS and PointClickCare software system preferred.
Familiarity with the Jewish religion, customs, and culture is considered an asset.

Bachelor's Degree (preferred)

Healthcare, preferably Long-Term Care: 1 year (preferred)
Office Administrative: 1 year (preferred)
work with vulnerable populations: 1 year (preferred)
